Conducting an FSA

In this video, we will cover the subject of conducting an FSA in SLM within SLM’s Functional Safety Assessment Module.

Fsas may be conducted and recorded using a guided system of steps to record the information required for Fsas per IEC 61511.

Chapter one will be creating a new FSA.

In Chapter 1, we’re going to cover how to create a new functional safety assessment and things to consider.

Fsas are created under either a unit or an SIS and may be linked to multiple units or Sis’s.

Functional Safety Assessments must be created from either the unit or the SYS level and can be created by clicking the Add FSA button in the top right corner of the FSA list table.

If the FSA is created from the unit level, the system will ask for one or more Syss to be selected and then click Next.

Now a stage must be selected so the system knows which checklist to display for the FSA.

Additionally, the self-assessment will be added to the FSA by default, so checking the box here will turn off the self-assessment for this FSA before saving to create the new FSA.

In Chapter 2, we’ll cover what a self-assessment is and how to record it, and things to consider.

FSA self assessments are optional to be recorded and do not negatively affect the FSA data itself.

If the self-assessment was left as default or selected to be displayed after creation, checklists in the Self-assessment tab will be displayed where the site personnel may enter information for equivalent but separate checklists that the independent FSA team will also use.

The purpose of self assessments is both to help the incoming FSA team with how the site personnel see the health of their functional safety systems and also to give the site personnel feedback on their own assessment of their functional safety habits.

The information entered into the Self-assessment checklist will be visible to the FSA team in the FSA checklist, but will not automatically fill out the FSA checklist comments.

In Chapter 3, we’ll cover information to record when setting up the FSA prior to executing and things to consider.

An FSA may cover multiple stages and may be changed after initial setup.

Initial information to be entered by the FSA team will include fields that are present in the Workflow, General Information and Participants tabs.

The Workflow tab will contain the information originally entered during the creation of the FSA, but can be changed at any time within this tab.

Stage numbers may also be changed at any time where selecting one or more stages will combine the checklists that are included in the selected stages into the FSA upon saving.

As previously mentioned, self assessments performed can be turned on or off at any time by selecting this button or deselecting it.

The General Information tab allows basic information about the assessment to be entered, such as the FSA’s background, the description, the date started, the company document number that will be given to the report, and the Participants tab will then allow the FSA team to document the people that will be participating in any way during conducting.

The FSA participants may be either added manually or from SLM personnel objects that exist within the same site already as the FSA and may be selected as FSA participants to add to the list.

Their role during the FSA may then be selected, and any notes for a participant may also be added.

Action items recorded here may be associated to specific checklist items.

FSA checklists will be automatically displayed for the FSA based on the stage that was selected during the FSA creation or in the Workflow tab, and the independent FSA team will fill out these checklists.

The self-assessment findings, if the self-assessment was left as default or selected to be displayed, will be seen alongside the FSA review columns.

In these FSA review columns, the FSA team will enter assessments for each item in the checklist, the conformance assessment score and when the item was verified.

The CAS or the Performance Assessment score may also be selected for each item and will accumulate these FSA findings into the KPI reports for the management level views.

If any action must be taken or noted for any item in the checklist, the green Add button to the left of the row may be clicked to add an action item to the FSA that references the specific checklist item.

In Chapter 5, we’ll cover recording additional FSA information and things to consider.

Information recorded in these sections may be added incrementally as an FSA execution progresses.

During execution of the FSA, the team will want to record things like additional questions and findings not identified in the checklist, interview records of site personnel, and overall key summaries of the various safety life cycle phases appropriate to the selected FSA stage.

Questions not covered by the checklist items may be recorded in the Additional Questions tab, where, like the checklist items, specific question items and finding and comments may also be recorded.

Interviews conducted during execution of the FSA may be recorded in the Interviews tab, with basic information recorded about the interviewee and notes about the interview, as well as action items again that may be recorded for the specific interview.

Finally, the FSA result findings may be entered by the FSA team to include summaries of the overall results and the date that the FSA was completed.

Any additional FSA action items may still be recorded here.

The Action Items tab will then accumulate all of the action items that have been recorded during the various parts of the FSA execution into one single table, where the FSA team may then review and or close the actions and hand them off to the site personnel upon final completion of the FSA.

The FSA Final Report will automatically update with all of the information entered in the FSA steps and after approval.

The FSA will not be editable unless the approval is removed.

After the FSA team has completed execution of the FSA, the final report will automatically be updated with all recorded information and allow for the FSA data to be printed to a pre formatted report.

Since this is all done by the system automatically, the FSA team is not required to enter any additional information in the Final Report tab.

With the FSA complete, the site personnel may then lock the FSA data from further editing by proving the FSA with the standard SLM approval functionality.
